About us:

Pathfinder was founded by clinicians to provide the best possible care to people with complex mental health needs. With the backing of social impact investors, Pathfinder has been able to reimagine how mental health services are designed and delivered.

We put the individual first in everything we do. This means placing their needs at the heart of all decisions, focusing on recovery and providing the most effective, least restrictive interventions.

Our Pathway of Care

 

Person-centred

As part of a pathway of care, we will provide person-centred inpatient care to people with severe enduring mental health conditions, co-morbid physical problems and forensic histories.

We have specialist high dependency beds to support individuals who may have greater needs due to aggression, agitation and other complex needs. 

Multidisciplinary

Under a recovery pathway, an expert multidisciplinary team consisting of Consultant Psychiatrists, Consultant Psychologists, Occupational Therapists, Nurses and Recovery Workers will support clients to live safe, independent lives. 

Referrals

We are able to offer a 24-hour rapid assessment. We accept referrals from the community, stepping down from secure settings or direct transfers from prison.

 

Ashness House, North London

 

Ashness House provides specialist assessment, treatment and support to stabilise a person’s symptoms and help them gain or regain the skills and confidence to live independently in the community. The unit will have a high dependency and rehabilitation beds providing a pathway of care.

Ashness House specialises in the rehabilitation of working age men with severe and enduring mental health illness, co-morbid psychiatric disorders and physical health problems.

 

A whole-person approach to care and treatment delivered by an expert multi-disciplinary team will facilitate assessment, monitoring and review of both mental and physical wellbeing.  With a clear pathway, the goal is to move on to community rehabilitation.

All support and risk plans are assessed and overseen by expert and experienced mental health professionals. Our recovery-focused therapeutic approach is overseen by Consultant Psychologists, Psychiatrists, Specialist Nurses and Occupational Therapists.
